LOUIS ? Virtually every day, a St. Louis County policeofficer takes a mentally ill person to the St. Louis MetropolitanPsychiatric Center for care.It provides treatment not available in a jail and offersservices better suited to a psychiatric incident than an emergencyroom, according to county police Sgt. Barry Armfield. He said Tuesday he fears that option may vanish. The Missouri Department of Mental Health plans to close thecenter July 1 to emergency and acute-care psychiatric patients, aspart of its plan to save the cash-strapped state about $7million.Mental health workers and advocates protested outside thefacility at 5351 Delmar Boulevard in April, saying the eliminationof emergency psychiatric care could lead to increases inhomelessness, crime and hospital overcrowding. Now Gucci Indy Bags, the St. Louis Area Police Chiefs Association is pleadingwith the private sector and community leaders to save thecenter."We take the hardest-to-serve and most noncompliant there,"Armfield explained. "These people aren't going to go away (just)because they close the MPC. They'll still be there."Across the Missouri side of the region, police officers handleabout 200 incidents a month involving the mentally ill. About 89percent end with trips to emergency rooms, including MPC.Police do take some to hospitals, especially if they haveinsurance, family support or doctors there already, Armfieldsaid."If they don't have insurance, the hospitals have to eat thatcost," he said. The people in need, "have a better chance at MPC ofbeing treated longer and getting outpatient services."The typical acute psychiatric patient has a mental illness suchas bipolar disorder or schizophrenia, comes to the facility in acrisis and is usually released within 10 days, after adjustments tomedications.About a year ago, two similar state-run facilities elsewhere inMissouri were saved. In Columbia, the University of Missouri now runs all operationsat the Mid-Missouri Mental Health Center. In Kansas City, Truman Behavioral Health took over emergency andacute care at the Center for Behavioral Medicine, leaving theDepartment of Mental Health to oversee longer-term care beds. Thatarrangement has worked "very well," said Marsha Morgan, chiefoperating officer for behavioral health at Truman MedicalCenter. The hospital's share of the beds has been running at about 96percent occupancy. Morgan expects to break even on costs."For us, it made sense," Morgan said. "It was an opportunity toget people who were mentally ill out of our medical emergencydepartment and get them to where psychiatrists, psychiatric nursesand social workers are, so they could get appropriate care and giveour medical emergency department a chance to focus on traumas andother emergencies."In St. Louis, members of the Missouri Hospital Association planto talk with state officials on Thursday about the impendingclosure of MPC, said Laurent Javois, regional executive officer forthe Department of Mental Health."I'm still hopeful that a community partner will come forward... But if the community remains silent on this, it's likely thatno partner will come forward," Javois said.Dr. John Lynch, chief medical officer at Barnes-Jewish Hospital, said he is not aware of any movement to transfer MPC's operationsto BJC HealthCare.SSM Health Care has no plans to take over operations at MPC,said John Eiler, executive vice president for behavioralhealth."We already have about 30 percent of the patients that are beingtreated in any hospital for psychiatric illness," Eiler said. "Wereally don't have the capacity to open new beds or pick up morecapacity to meet this community need."Last year, SSM looked into leasing some operations at the MPC Gucci For Sale, but decided the facility was too far from SSM's other hospitals toprovide coordinating medical care. The agreements in Kansas City and Columbia are between hospitalsin the same vicinity, Eiler noted.St. Louis-area hospitals have already seen an increase in mentalhealth patients, including transfers from police departments andjails, as MPC has gradually reduced its capacity in recent years,Eiler said.Armfield said police officers find that those who fit theircriteria for treatment at MPC tend to fare better than they wouldafter a short stay at a hospital."They have the relationships and know |

Read more Sister Mary Carolyn McQuaid now sees the blood that came fromher right breast in February as a blessing. She'd had a mammogram four months earlier that did not detectanything suspicious, she said. Her next mammogram would have beeneight months later."I knew (the nipple discharge) wasn't good Gucci Hats, and saw my internista week later," said McQuaid, 64, of the Sisters of the GoodShepherd Convent in St. Louis. "She said how lucky I was to be inSSM System because she could send me to see Dr. Aislinn Vaughan"for a ductal endoscopy. The procedure revealed stage 1 breast cancer. Most women who have breast cancer don't have bloody nippledischarge. And only 5 percent to 10 percent of those who experiencesuch a discharge are diagnosed with breast cancer. But Vaughan, abreast surgeon at Breast Care at St. Joseph Health Center in St.Charles, usually recommends checking it out, all the same.It could mean the difference between finding cancer when it'scontained in a milk duct, and later, when it's spread to breasttissue or beyond.October is Breast Cancer Awareness month, a time when healthcare experts and nonprofit groups drive home the message that earlydiagnosis equals better outcome. And ductal endoscopy (or mammaryductoscopy, as it's also called) can detect cancer as early asstage 0, when it's fully contained in the duct.Though the procedure has been around for nearly a decade,Vaughan is one of just three area doctors who perform it.Dr. Amy Cyr and Dr. Julie Margenthaler, breast surgeons atWashington University, also do ductoscopies. All three studied the technique under Dr. Jill Dietz, formerprogram director for the breast surgical fellowship at WashingtonU. Dietz brought the technology to St. Louis from Cleveland Cliniceight years ago; she moved back there in 2007.The procedure is minimally invasive and allows the doctor tomore efficiently find the lesion or growth causing bleeding in themilk duct, biopsy it and, if needed, remove it. Eighty percent ofall breast cancers begin in the ducts, and most are detected usingmammograms, which Vaughan says is still the best form of breastscreening Gucci Briefcases.'Colonoscopy of the breast'Milk ducts run like tree branches through the breast andculminate in 15 openings in the nipple. The widest ducts, near thenipple, are 1.5 to 2 mm in diameter. They become microscopic towardthe back of the breast near the chest wall.During a ductal endoscopy, the doctor is able to see the liningof the ducts by threading a fiber optic, flexible scope into one ofthe nipple openings and through the leaky duct. The scope, which is encased in a 6-inch long hollow thin wire, magnifies tissue up to 60 times and projects images onto a computerscreen. It also collects cells for biopsy. The fiber optic scopeemits a glow, like the one on the tip of E.T.'s finger in theclassic Spielberg film, Vaughan said.Patients are put under general anesthesia for the procedure."It's sort of like a colonoscopy of the breast Gucci Heritage," Vaughansaid. The ductoscopy was quick and painless for McQuaid; she was backto work two days later. Then the biopsy results came: The tumor had moved out of theduct and invaded surrounding tissue and a lymph node.Vaughan gave McQuaid several options: she could do a lumpectomywith radiation, a mastectomy or a mastectomy with reconstruction.Meanwhile, a mammogram on her left breast came back lookingsuspicious.McQuaid had a double mastectomy and five lymph nodesremoved.challenging procedureThough the nipple discharge symptom is rare, Vaughan estimatesshe's done about 50 ductoscopies in the past four years."It's challenging to do because it's a very small opening thatyou're working with, and it's done in the dark," she said.If Vaughan finds the lesion and it's malignant, she makes anincision on the outer edge of the areola and removes the duct andany affected tissue. The ductoscopy's three-dimensional imagingshows its precise boundaries."With this we remove less tissue because it allows us to be somuch more accurate," she said. When cancer cells are close to the edge of the duct or havespread to surrounding tissues, Vaughan will do a second surgerycalled a re-excision or partial mastectomy to remove more |

Although the action was in "poorjudgment," it did not violate city policies, Schroeder said. The other six allegations "identified the shortcomings on thepart of Mr. Draper," he said Gucci Heritage. One was that Draper changed criteria for the selection of anengineering firm for a road project and asked a management intern,Kristin Gendron, to lie about it.Gendron had been asked to rate engineering firms for theSunshine Drive project.
